What To Bring To Your Live Scan Appointment

1. A COMPLETED “Request for Live Scan Service” form in triplicate, which should be provided by your employer or school. (We have provided a link to this form, BCII-8016, Here for your convenience.) This form is blank and provided in Adobe PDF format and requires Adobe Acrobat Reader to be installed on your computer.

2. Your valid picture identification. The following are acceptable forms of identification:

· Valid, state issued identification or driver's license.

· Driver's license issued by a Canadian government authority.

· Non-expired ID card used by federal, state or local government agencies/entities with a photograph (i.e., passport, visa, etc.).

· Military, military dependent's ID card or U.S. Coast Guard Merchant Mariner Card

· Alien Registration/Immigration/Green card.

3. Payment. We accept Cash, Money Orders, Credit, Debit or Business Checks and gladly offer Account Billing! Please make checks payable to Live Scan Solutions for the appropriate amount. *Note: if you are uncertain about the exact amount, please wait to fill out the check. Thank you.
